In what order should I submit the Counselor school forms?

School forms must be filled out and submitted in the following order:

  • School Report
  • Counselor Recommendation
  • Mid Year Report and/or Optional Report
  • Final Report

The Counselor Recommendation, Optional, Mid Year, and Final Report forms are intended to be used as a way of updating colleges about a student's academic progress subsequent to submitting the initial School Report form.  You will not see the option to submit subsequent forms until after the School Report has been completed and submitted.

Counselors should submit Mid Year Reports as soon as first semester/trimester grades are available. The Common App Online School Forms system does not send reminders about these forms, nor do counselors receive invitations to submit as they do with the Secondary School Report. It is the student's responsibility to keep track of this requirement and ensure that the counselor is aware of it.
